Data Protection and Privacy

Each time you visit the doctor at Highfield Park Medical Centre an entry will be recorded in your own personal medical record held on our electronic database. This medical record is a confidential document and password protected. It is the policy of this clinic to maintain security of personal health information at all times and to ensure that this information is only available to authorised doctors.  All staff and health professionals that work at Highfield Park Medical Centre are bound by confidentiality agreements. 

We comply with the Privacy Act 2000 and the Victorian State Act 2001 including the way we store, use and disclose health information. The contents of your health records will be divulged only with your consent or where justified by law. You will need to make an request in writing to see your medical record at the clinic. Some of the details of your medical records may be disclosed to other medical or health care professionals who are either providing you with treatment, tests or services such as life insurance or driving medicals etc.

Transfer of your medical records

If you would like us to request a copy of your previous health records from another doctor or hospital we have authorisation forms for you to complete and sign. We will fax these completed forms to your previous doctor. We do not charge for transferring Health Summary records. 

New Patient forms

New patients please arrive 10 minutes prior to your first appointment to complete the necessary paperwork and allow entry of your details into our computer system. We will need your name and address and contact telephone numbers, name and contact number for your next-of-kin, occupation, Medicare number, pension or health-care-card details.
